Not official yet. These are projections from the 2026 Medicare Trustees Report (released June 9, 2026), as also reported by AARP and KFF. CMS usually publishes the official 2027 Part B premium and deductible in mid-October to mid-November: the 2024 figures came out October 12, 2023, the 2025 figures on November 8, 2024, and the 2026 figures on November 14, 2025. We will update this page the day they are released.

Medicare Part B is the part of Original Medicare that covers doctor visits, outpatient care, preventive services and durable medical equipment. Most people pay a monthly premium for it, and that premium resets every January. The Centers for Medicare & Medicaid Services (CMS), the federal agency that runs Medicare, sets the new amount each fall.

Right now, the only formal estimate of the 2027 premium comes from the Medicare Trustees, the board that reports each year on the program’s finances. So, below we lay out the projected Medicare Part B premium 2027 figures, the official 2026 numbers for comparison, how IRMAA (the income-related surcharge) will change, and how the 3.6% Social Security raise fits in.

Medicare Part B Premium 2027: What Will You Pay?

The projected Medicare Part B premium 2027 amount is $209.50 a month. The Trustees Report states that “the estimated monthly premium for 2027 is $209.50,” compared with $202.90 in 2026 and $185.00 in 2025.

In other words, the standard premium would rise by $6.60 a month, or about $79 a year. That is a much smaller jump than the one people saw for 2026, when the premium climbed $17.90 (9.7%). However, the Trustees themselves call these values estimates and say the actual amounts “are likely to be somewhat different as experience emerges.”

Why is the 2027 increase smaller than 2026?

The Trustees Report does not give a single reason for the smaller 2027 estimate, so it helps to look at what drove 2026. For 2026, CMS said the increase came mostly from projected price changes and higher expected use of services, according to its 2026 premium fact sheet. CMS also said the 2026 premium would have been about $11 higher without its action to cut spending on skin substitutes.

Even so, the longer-term trend still points up. The same Trustees table projects $224.50 for 2028 and $272.10 for 2031.

How accurate is the Medicare Part B premium 2027 projection?

The projections are a useful guide, but they are not a guarantee. For example, last year’s Trustees Report projected $218.60 for 2027, which is $9.10 higher than this year’s $209.50 estimate, according to MOAA’s summaries of the two reports. AARP also notes the Trustees hit the actual figure exactly only three times in 10 years. Meanwhile, KFF puts the projected 2027 rise at about $79 a year, which matches the Trustees number.

What Will the Medicare Part B Deductible Be in 2027?

The projected 2027 Part B deductible is $292 a year, up $9 from the official 2026 deductible of $283. You pay this amount out of pocket each calendar year before Part B starts sharing costs.

After you meet the deductible, you typically pay 20% of the Medicare-approved amount for most Part B services. Because the deductible is indexed to average per-person Part B spending, it tends to rise at roughly the same pace as the premium. As a result, the Trustees project the deductible will reach $313 in 2028 and $379 in 2031.

What about Part D costs in 2027?

Several 2027 Part D (prescription drug) numbers are already set. The Trustees Report lists the 2027 standard Part D deductible at $700 and the out-of-pocket threshold at $2,400, and it marks both as already finalized. In addition, CMS said on September 28, 2026, that the average stand-alone drug plan premium is projected to rise from $35.09 to about $36 a month in 2027, per its 2027 plan announcement.

What Are the IRMAA Brackets for 2027?

CMS has not published official 2027 IRMAA brackets yet. Until it does, the best guide is the official 2026 table plus the Trustees’ projected 2027 surcharge amounts. IRMAA, the income-related monthly adjustment amount, is an extra charge on top of the standard premium for people with higher incomes.

Projected 2027 IRMAA amounts for Part B

Next, the Trustees Report (Table V.E3) projects the 2027 surcharge for each of the five IRMAA tiers. We added each surcharge to the projected $209.50 standard premium to show the projected total. The 2027 income ranges are not out yet, so the table labels each tier by its share of program cost instead.

How do the 2027 IRMAA brackets adjust?

The income thresholds rise with inflation each year. Under federal law (42 U.S.C. 1395r(i)), CMS increases each dollar threshold by the change in the average Consumer Price Index for all urban consumers (CPI-U) over the 12 months ending in August, and then rounds to the nearest $1,000.

  • Lower tiers: the $109,000 / $218,000 starting point and the next three breakpoints will likely move up for 2027, but the exact amounts are not official yet.
  • Top tier: the $500,000 / $750,000 threshold is not indexed until 2028, so it should stay the same in 2027, according to the statute and the Trustees Report.
  • Income year: 2027 IRMAA uses your 2025 tax return, because the law looks back two years.
  • Part D IRMAA: uses the same income brackets; the 2027 Part D surcharge amounts are not out yet.

Social Security, which decides who owes IRMAA, says it uses “the most recent federal tax return the IRS provides.” You can ask it to lower IRMAA after a life-changing event, such as retirement, divorce or the death of a spouse, per the SSA IRMAA page.

Pro tip. If you retired in 2025 or 2026 and your income fell, file Form SSA-44 with documents showing the event. Because IRMAA uses income from two years back, a recent drop in income may not show up in your premium otherwise.

How Does the 3.6% COLA Interact With the 2027 Premium?

The 2027 Social Security COLA is 3.6%, and it is official. The Social Security Administration (SSA) announced it on October 14, 2026, and said retirement benefits will rise by $75 a month on average, starting in January 2027, per its press release.

Most people have their Part B premium taken directly out of their Social Security check. So the January 2027 deposit will show both changes at once: the COLA raise minus the higher Part B premium. If the Medicare Part B premium 2027 projection holds, the increase of $6.60 would eat about 9% of a $75 raise.

Example (illustration only)Amount
Monthly benefit before the COLA$2,000.00
3.6% COLA raise+$72.00
Projected Part B premium increase-$6.60
Net monthly gain in 2027 (projected)+$65.40
Our arithmetic using the official 3.6% COLA and the Trustees’ projected $6.60 premium increase. Your numbers will differ.

What is the hold-harmless rule?

The hold-harmless provision keeps your net Social Security check from shrinking because of a Part B premium increase. According to the Trustees Report, it limits the dollar increase in your premium to the dollar increase in your Social Security benefit.

However, it covers only people whose premiums are deducted from their Social Security benefits, roughly 70% of Part B enrollees. People who pay IRMAA, and people who do not get Social Security checks, are not covered. With a 3.6% COLA and a projected $6.60 premium increase, the rule would only matter for very small benefits (below roughly $184 a month), so most people will not need it in 2027.

When will you see your 2027 premium?

SSA says the 2027 Medicare premium amount will appear in the my Social Security Message Center starting in late November. If you have not opted in to online notices, your COLA notice arrives by mail in December.

What Should You Do During Open Enrollment?

Use Medicare Open Enrollment, which runs from October 15 to December 7, 2026, to review your coverage for 2027. Changes take effect January 1, 2027, and your plan must get your request by December 7, according to Medicare.gov.

The Part B premium itself is the same whichever plan you choose, so open enrollment will not lower it. Instead, the savings come from choosing the right Medicare Advantage plan or Part D drug plan around it.

  • Compare drug plans: premiums and covered drugs change every year, so check your list on the Medicare Plan Finder.
  • Check Medicare Advantage: you generally still pay the Part B premium if you join a Medicare Advantage plan, so compare the plan premium, deductible and network on top of it.
  • Switch if needed: you can move between Original Medicare and Medicare Advantage during this window.
  • Budget for the deductible: plan for the projected $292 Part B deductible in January.

Watch out. Be wary of unsolicited calls or ads that promise to lower your Part B premium. CMS sets the standard premium for everyone, so compare plans yourself on Medicare.gov or call 1-800-MEDICARE before you give out your Medicare number.

When Will CMS Announce the Official 2027 Premium?

CMS has not set a public date, but it usually announces the next year’s Part B premium between mid-October and mid-November. As of October 23, 2026, the CMS newsroom had not posted the 2027 numbers.

Sources: CMS premium fact sheets for 2024, 2025 and 2026; 2026 Medicare Trustees Report. Checked October 23, 2026.

Separately, CMS paid a one-time $90 Part B premium rebate in October 2026 to about 20.8 million Original Medicare enrollees who do not pay IRMAA or get Medicaid premium help, per its rebate FAQ. That rebate applies to 2026 only and does not change the 2027 premium projection.

Our verdict

Plan on a Medicare Part B premium 2027 cost of about $209.50 a month and a $292 deductible, but treat both as projections until CMS confirms them. For most retirees, the 3.6% COLA should comfortably cover the projected $6.60 increase. Higher earners should check their 2025 income against the IRMAA tiers, because the surcharge grows faster than the standard premium.

Meanwhile, the most useful step right now is reviewing your drug or Medicare Advantage plan before December 7. We will update this page with the official 2027 figures the day CMS releases them.

Frequently Asked Questions

How much will Medicare Part B cost in 2027?

The standard Medicare Part B premium 2027 figure is projected at $209.50 a month, according to the 2026 Medicare Trustees Report. That is $6.60 more than the official 2026 premium of $202.90. Higher earners will pay more because of IRMAA. CMS has not announced the official amount yet, so this figure could still change before January 2027.

Will Medicare Part B go up in 2027?

Yes, it is expected to. The Trustees project the standard premium will rise about 3.25%, from $202.90 to $209.50 a month, and the deductible from $283 to $292. This would be a much smaller increase than the 9.7% jump for 2026. The official 2027 numbers usually come from CMS in October or November.

What are the IRMAA brackets for 2027?

Official 2027 IRMAA brackets have not been released. In 2026, IRMAA starts above $109,000 of income for single filers and $218,000 for joint filers. For 2027, those thresholds rise with CPI-U inflation, except the top tier of $500,000 and $750,000, which stays the same until 2028. The 2027 surcharge will be based on your 2025 tax return.

What will the Medicare Part B deductible be in 2027?

The 2027 Part B deductible is projected at $292 a year, up from the official 2026 deductible of $283, according to the 2026 Medicare Trustees Report. You pay the deductible before Medicare begins paying its share. After that, you usually pay 20% of the Medicare-approved amount for most Part B services. CMS will confirm the final figure this fall.

When will the 2027 Medicare premiums be announced?

CMS has not given a date. However, recent announcements have come between mid-October and mid-November: October 12, 2023 for the 2024 premium, November 8, 2024 for 2025, and November 14, 2025 for 2026. SSA also says your personal 2027 premium will appear in your my Social Security Message Center starting in late November.

Will the 3.6% COLA cover the 2027 Part B increase?

For most people, yes. The official 2027 COLA of 3.6% raises the average retirement benefit by about $75 a month, while the Part B premium is projected to rise only $6.60. If you pay IRMAA, however, your surcharge also rises, so your net gain will be smaller. The hold-harmless rule protects many enrollees whose increase exceeds their raise.
